The following forms are the most commonly used in the Juvenile Court:

CASA Volunteer Application--(PDF)
Used by those wishing to volunteer for the CASA program.
PETITION - Delinquency Proceedings: School Truancy/Misconduct--(PDF)
Used to file a truancy/misconduct petition in the Juvenile Court.
